Notice of Privacy Practices
This notice describes how health information about you may be used and shared, and how you can get access to it. Please read it carefully.
Who this notice covers
This notice applies to True Chiropractic San Diego, our providers, employees, students and volunteers, and to any business partners who handle your health information on our behalf under written agreements. It covers records we create or receive about your care, your bills and your visits.
Our duties
We are required by law to keep your protected health information private, to give you this notice, and to follow the version of it that is currently in effect. If a breach of your unsecured health information occurs, we will notify you as the law requires.
How we may use and share your information without your written authorization
- Treatment. To examine you, plan and provide care, and coordinate with other providers involved in your care, such as your primary physician, a specialist we refer you to, or an imaging center.
- Payment. To bill you, your insurer, a third-party payer or an attorney handling your claim, to confirm coverage, and to collect what is owed.
- Healthcare operations. To run the practice — quality review, staff training, scheduling, audits, licensing and business planning.
- Appointment reminders and care information. To remind you of visits by phone, text, email or mail, and to tell you about treatment options or services we offer that may help you. You may ask us to use a different method or stop these contacts.
- People involved in your care. To a family member, friend or caregiver you identify, to the extent they are involved in your care or payment, unless you object.
- Required by law. When federal, state or local law requires it.
- Public health and safety. To report disease, injury, adverse events, or to prevent a serious threat to someone's health or safety.
- Abuse, neglect or domestic violence. To an authorized agency when we reasonably believe a person is a victim.
- Health oversight. To agencies that license, audit or investigate the health system.
- Legal proceedings. In response to a court order, subpoena or lawful request, with the protections the law requires.
- Law enforcement. For limited purposes such as identifying a suspect or victim, or reporting a crime on our premises.
- Workers' compensation. As permitted for work-related injury claims.
- Coroners, medical examiners, funeral directors and organ donation. As permitted by law.
- Research. Only with approval from a review board that protects your privacy, or with your authorization.
- Military, veterans, national security and correctional institutions. In the limited situations the law allows.
- Business associates. To vendors who perform services for us — such as billing, IT, records storage, online scheduling and patient communication — under contracts that require them to protect your information.
Uses that require your written authorization
We will ask for your written permission before using or sharing your information for marketing paid for by a third party, before selling your information, before sharing psychotherapy notes where they exist, and for any other purpose not described in this notice. You may cancel an authorization in writing at any time; the cancellation does not affect what we already did while it was in effect.
Reviews, photos and testimonials
We will not post your name, image or story on our website or social media without your separate written consent.
Your rights
- See and copy your records. You may inspect or request a copy of your health information, on paper or electronically. We may charge a reasonable, cost-based fee. In limited cases we may deny access; if so, we will explain why and how to request a review.
- Request a correction. If you believe information is wrong or incomplete, you may ask us to amend it. We may decline in some cases and will tell you why in writing.
- An accounting of disclosures. You may ask for a list of certain disclosures we made in the six years before your request, other than those for treatment, payment, operations, or ones you authorized.
- Request restrictions. You may ask us to limit how we use or share your information. We are not required to agree, except that if you pay for a service in full out of pocket and ask us not to share it with your health plan, we will honor that request unless the law requires the disclosure.
- Confidential communications. You may ask us to contact you at a specific phone number, address or by a specific method. We will accommodate reasonable requests.
- A paper copy of this notice. Even if you agreed to receive it electronically, you may request a printed copy at any time.
- Choose someone to act for you. A person with medical power of attorney or legal guardianship may exercise these rights for you; we will confirm their authority first.
To exercise any of these rights, contact our Privacy Officer using the information below.
Changes to this notice
We may change this notice and our privacy practices. Any new notice will apply to all health information we hold, including information created before the change. The current version is posted in our office and on www.aloha-domain1.com, with its effective date.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington, DC 20201, by phone at 1-800-368-1019, or online at https://www.hhs.gov/ocr/complaints.
